I agree to have and pay for the above procedure performed at Animal Medical Center of Panola County. I understand that all precautions will be taken to ensure the well-being of my pet, but with anesthesia and surgery, complications may arise. I understand this consent form will be placed in the permanent medical records.
1) I understand that my pet will be given an examination prior to anesthesia. I am also aware that a good physical examination does not always reveal abnormalities that can be detected by other methods (e.g. bloodwork). Preanesthetic bloodwork is recommended by the veterinarians at AMC to help in preparation of anesthetic protocols.
2) Pain management will be determined by the attending veterinarian
3) Many surgical procedures can be performed using a CO2 Laser which decreases bleeding, decreases pain, and enhances healing by decreasing tissue damage.
4) Many surgical procedures can be treated with a Therapy Laser which helps your pet to heal faster post-operatively. Laser assists in pain management by releasing endorphins and stimulating injured cells to heal at a faster rate.
5) Microchips can easily be placed while a pet is under anesthesia
6) Histopathology, microscopic examination of surgically removed tissue, can be sent to a diagnostic lab to determine the type of tumor/tissue present.
In accordance with state and county law, all animals 4 months old and older are required to be current on Rabies vaccination. If the vaccination is not current, one will be administered. Other core vaccines such as Distemper -Parvo and Bordetella are required while in the hospital for all patients protection against these deadly diseases.
If the patient is found to have any infectious parasites, zoonotic or vector borne diseases, Animal Medical Center reserves the right to treat at my expense to minimize the risk of infecting other patients in the hospital.
